Abstract

Abstrak

Latar Belakang: Methicillin-resistant Staphylococcus aureus (MRSA) resisten terhadap antibiotik golongan betalaktam sehingga dapat meningkatkan angka morbiditas dan mortalitas di rumah sakit. Lama bertugas di ruang perawatan intensif seperti High Care Unit (HCU) menjadi salah satu faktor risiko terjadinya kolonisasi MRSA pada perawat. Perawat yang memiliki kolonisasi MRSA meskipun tidak bergejala (carrier) tetap berisiko tinggi untuk terinfeksi MRSA dan juga berisiko menyebabkan kolonisasi dan infeksi MRSA pada pasien dan tenaga medis yang lain.

Objektif: Penelitian ini bertujuan untuk mengetahui perbedaan carrier MRSA pada rongga hidung perawat berdasarkan lama bertugas di HCU Interne RSUP Dr. M. Djamil Padang tahun 2024.

Metode: Jenis penelitian adalah analitik observasional dengan desain cross sectional dengan total 20 sampel nasal swab perawat yang bertugas di HCU Interne melalui teknik total sampling. Setelah itu dilakukan pemeriksaan Gram, uji katalase, dan uji alat Vitek-2 kemudian dianalisis menggunakan uji Fisher exact.

Hasil: Hasil penelitian didapatkan prevalensi kolonisasi MRSA sebesar (20%) dengan distribusi kolonisasi MRSA tertinggi pada kelompok lama bertugas (≤3 tahun) sebesar (21,4%) dibandingkan dengan lama bertugas (>3 tahun) sebesar (16,7%). Namun secara statistik menunjukkan tidak terdapat perbedaan signifikan antara kolonisasi bakteri MRSA berdasarkan lama bertugas di ruang HCU Interne (p = 1,0).

Kesimpulan: Sebagian besar kolonisasi MRSA ditemukan pada perawat dengan lama bertugas di HCU Interne (≤3 tahun) meskipun secara statistik tidak terdapat perbedaan secara signifikan. Peningkatkan upaya pengendalian infeksi diperlukan untuk memutus penyebaran MRSA di rumah sakit.

Abstract

Background: Methicillin-resistant Staphylococcus aureus (MRSA) is resistant to beta-lactam antibiotics, which can increase morbidity and mortality rates in hospital. Length of service in intensive care units such as the High Care Unit (HCU) is one of the risk factors for MRSA colonization in nurses. Nurses who have MRSA colonization even though they are asymptomatic (carriers) remain at high risk of MRSA infection and are also at risk of causing MRSA colonization and infection in patients and other health workers.

Objective: This study aims to determine the description of MRSA colonization in the nasal cavity of nurses based on length of service in the HCU Interne.

Methods: This study is an observational analytical study with a cross-sectional design with a total of 20 nasal swab samples of nurses working at the HCU Interne through a total sampling technique. After that, Gram examination, catalase test, and Vitek-2 tool test were carried out and then analyzed using the Fisher exact test.

Results: The results of the study showed that the prevalence of MRSA colonization was (20%) with the highest distribution of MRSA colonization in the group with long service (≤3 years) amounting to (21.4%) compared to those with long service (>3 years) amounting to (16.7%). However, statistically it shows that there is no significant difference between MRSA bacterial colonization based on length of service in the Internal HCU room (p = 1.0).

Conclusion: Most of the MRSA colonization was found in nurses who had served in the HCU Interne for a long time (≤3 years) although there was no statistically significant difference. Increased infection control efforts are needed to stop the spread of MRSA in hospitals.
